Fill Out New Patient Paperwork—To save time at your appointment, you may either use our Patient Portal to fill out Patient Medical History or print the paperwork below, fill it out, and bring it to your next appointment.

Check Insurance Information—Lifestyle Eye Center accepts most medical insurance plans, but it is always wise to check with yours prior to scheduling to confirm coverage.

 
 
 

YOUR APPOINTMENT

Upon Arrival—When you arrive for your appointment, make sure to be around 15 minutes early. Don’t forget to present your photo ID and insurance card to receptionist upon arrival.

Late Arrivals—If you are running late, give us a call and we will see what we can do about keeping your original appointment. If you need to reschedule, please notify us 24 hours in advance and we will reschedule you at your earliest convenience. 

Wait Time—At times, our patients have emergencies that may result in longer waiting times than we prefer. If we are running behind and you are unable to wait, we will gladly reschedule you for the next available appointment. We apologize for any inconvenience this may cause you.

AFTER YOUR APPOINTMENT

Medication Refills—For a timely refill, please request medication refills at least three days before your prescription is gone or has expired. If we have referred you to your regular eye doctor or physician, please contact their office for prescription refills. 

Make a Payment—Call the office to make a payment over the phone.
